| Forwarding Remains to other Funeral Home This charge includes removal of remains, basic service of staff, embalming, necessary authorizations and local transportation. It does not include visitation, rites or ceremonies prior to forwarding of the body. This charge does not include a shipping container or transportation to airport.

| Receiving Remains from other Funeral Home This charge includes local pick-up of remains transportation of remains to the cemetery and basic services of staff.

| Immediate Burial This charge includes removal of remains, local transportation to the cemetery, necessary authorizations and basic services of staff. Does not include use of facilities or staff for any visitation. or ceremony, public or private, prior to burial.

Direct Cremation Includes removal of the body from place of death, local transportation to crematory necessary authorizations and basic services of staff. Does not include crematory fee or use of facilities or staff for any visitation or ceremony. If selected you must use an alternative container to encase the body. This can be made of fiberboard or composite materials.

Outer Burial Containers In most areas of the country, state or local law does not require that you buy a container to surround the casket in the grave. However, many cemeteries require that you have such a container so the grave will not sink.

| Embalming Except in certain special cases, embalming is not required by law. Embalming may be necessary however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

Visitation Service (per day) Our services will include visitation setup, use of the visitation room. Placement of the encased remains, display of floral arrangements, and supervision and attendance of visitation.

Funeral Service at Funeral Home Our services include coordinating the funeral arrangements, supervision of funeral and setup and use of facilities for funeral ceremony.

Funeral Service at Other Facility Our services include coordinating the funeral arrangements and equipment and staff to conduct the funeral at other facility.

Memorial Service Our services include coordinating the memorial service, supervision of the memorial service, staff to attend the memorial service and setup for the memorial service.

Graveside Service Our services include coordinating the funeral arrangement at graveside, supervision of the graveside service, and equipment and staff necessary to conduct the graveside service.
